Patient & Minor Safety Policy

For the safety of all patients and to minimize liability, our office requires that all minors be properly supervised at all times.

Children under the age of 10 may not be left unattended in the reception area or anywhere in the office while a parent or guardian is receiving treatment.

If you have an appointment, please do not bring children under 10 unless they are accompanied by another responsible adult who can supervise them at all times.

The use of iPads, phones, or other personal electronic devices does not constitute supervision and is not an acceptable alternative.

Patients under the age of 18 must be accompanied by a parent or legal guardian for their appointment. Minors may not be dropped off or seen without a parent or legal guardian present.

Failure to comply with this policy may result in cancellation or rescheduling of your appointment. All applicable cancellation or missed appointment fees will be charged in accordance with our office policy.
